Physical Forms

All student-athletes grades 6-12 MUST have the following forms completed and on file in the WCS Athletics Office. If the forms are not on file, sports participation is not allowed for any team at any level. Please click on the links below to download and print the form.

1. WCS Student-Athlete Participation and Parental Approval Form

2. FHSAA Preparticipation Physical Evaluation (EL2)

3. FHSAA Consent and Release from Liability Certificate (EL3)

** Form 1 MUST be notarized (Janet Bigelow in the HS office is a notary).

** Form 2 MUST have a Dr.'s signature.
