For students with prescription medications, your child’s physician must complete an Authorization for Medication form. This form includes medications administered by the nurse or carried by the students for self-administration. Additionally, students who require emergency medications must have an action plan completed by their prescribing physician. Please note, medication will not be accepted without a physician signed Authorization and/or action plan forms. To access these forms, log in to myWCS, click on the Resources tab, and then click on the Health Clinic resource board. We kindly ask that you do not delay in getting your child’s forms completed.

Please arrange to drop off medication, supplies, and orders at the Health Clinic the week of August 7, 2023. Each grade level will have a designated drop-off day and time, which will be communicated to you in advance. It is important to note that medications and orders will no longer be accepted on the day your child departs for Warrior Week.
